• Required adolescent vaccinations:

  • Tdap: Proof of 1 dose 

  • Meningitis vaccine:  2nd dose if 1st dose was received before 16th birthday 

• HealthCheck/physical examination from a doctor/medical provider

• Dental examination

There is no provisional enrollment for the required vaccinations. Proof must be provided before the students first day of school.

Health Check and Dental exams are valid if completed within one year of the first day of school. There will be a grace period of 45 days from the first day of school for students to provide proof of the completed HealthCheck and dental exams.
